Examples include:
HAZOP Closeout ahead of Miran Early Production Facility startup, where lethal levels of H2S were present in the well fluid. Key tactic was to triage HAZOP actions and insist that the ‘red’ ones (immediate risk of death) were closed out ahead of startup.
Process Safety audit (based on CCPS pillars) of TaqTaq site following a PS incident leading to burns injuries. Significant gaps highlighted including Emergency Lighting and ESD and Control Loops
